Pflegekraft im warmen Gespräch mit älterem Patienten
For referrers · Out-of-hospital ventilation

Hand over ventilated clients safely – without paperwork burden, coordination stress or deduction risk.

We are the extended arm of your discharge management. A designated transition coordinator takes care of prescription preparation, care organisation and feedback – for every ventilated or tracheotomised client.

The starting point

Since 2023, the ventilation transition has become a real bottleneck

Three legal developments have noticeably made the transition of ventilated clients harder – and this is exactly where our service package comes in. We turn these hurdles into plannable, secure processes.

Stricter AKI prescription

Since October 2023 only under the AKI Guideline (§ 37c SGB V) – more demanding, more error-prone and only by especially qualified physicians. Faulty prescriptions are returned and cost weeks.

Mandatory potential assessment

Before almost every prescription, a physician must check (forms 62A/62B) whether weaning or decannulation is possible. There are too few qualified physicians for this – the transition stalls.

Money risk

B-BEP deduction risk

Without a proper assessment of ventilation status and without prescribed follow-up care, invoice deductions are threatened (§ 9 Abs. 1a Nr. 8 KHEntgG). A poorly organised transition costs the hospital real money.

Overburdened discharge management

§ 39 Abs. 1a SGB V demands seamless follow-up care and aims to avoid the revolving-door effect – with around 13 approved forms per complex discharge. Time pressure, form pressure, financial pressure.

Our approach

Not just another provider.
An extended arm of your discharge management.

The core of our approach is not advertising, but relief. Whoever takes work, risk and uncertainty off your hands becomes the obvious choice – voluntarily, gladly and repeatedly.

“For every ventilated client, we take the form-filling effort, coordination burden, time pressure and financial deduction risk off discharge management – with a designated contact person and a closed feedback loop.”

The service package

Four levels of impact – from “takes work off your hands” to “becomes indispensable”

The deeper the level, the stronger the relief – and the more reliable the partnership. You choose how far we go.

1
Level 1 · The Start
Taking work off your hands
  • Prescription preparation optimized for approval. All forms (62A/62B, aids, medication, care) are professionally complete and aligned with the MD review logic – the physician's signature remains untouched.
  • Application, cost coverage and aids management all from a single source.
  • Pharmacy and medication management through our partner network.
2
Level 2 · The Quality
Providing security
  • Coordinated ventilation, tracheostomy and medication chain including legally required device instruction and training for family members – no dangerous breaking points.
  • First contact at the bedside before discharge – recording the ventilation status, getting to know each other, involving family members. Day X is not a leap into the deep end.
  • Open proof of quality (inspection results, certificates) as an anchor of trust.
3
Level 3 · The Promise
Guaranteeing reliability
  • Tiered service level: quick feasibility feedback, binding commitment to take over care, a transparent timeframe until care begins – instead of a blanket, risky 24-hour claim. Reliability beats speed.
  • Care is ready for the return home: ventilator, consumables, aids and medication ready for use at the place of care – if in doubt at the economic risk of the cooperation partners, so that no gap in care arises.
  • Rule-based, capped assumption of risk and advance payments – oriented exclusively towards the interests of the insured person.
  • 24/7 emergency and readmission concept against the revolving-door effect.
4
Level 4 · The Bond
Becoming indispensable
  • Designated transition coordinator as a permanent, directly reachable contact person – no hotline.
  • Orchestration of the potential assessment via a network of qualified physicians – we coordinate the appointments, we do not carry out the assessment ourselves.
  • Closed feedback loop: structured status feedback to your discharge management once the handover has taken place.
How the transition works

Five steps – one contact person

From the first call to the closed feedback loop to your discharge management: you report the case, we take care of the rest.

1

Your call is all it takes

You report the case to your transition coordinator – by phone or by fax/email. From here on, we take over the coordination.

2

First contact at the bedside

Recording the ventilation status, getting to know the client and relatives – before discharge, right at your facility.

3

Prescription & approval

We prepare all forms to optimise approval, coordinate the potential assessment and manage applications, aids and medication.

4

Care is in place

Device, supplies, medication and care team are ready for the return home – with no gap in care on day X.

5

Closed feedback

Your discharge management receives structured status feedback after the handover – the loop is closed.

Our homecoming promise

No gap in care on the day of homecoming – secured by a tiered, honest service level:

A

Fast feasibility feedback

You will quickly find out whether and how we can take on the case.

B

Binding acceptance commitment

A commitment from us is reliable – not a noncommittal "we'll have a look".

C

Transparent timeline

Until care begins, you know at all times where the case stands – honest rather than blanket 24-hour promises.

Your transition coordinator

One name, one face, one responsibility

No hotline, no changing contacts: a named transition coordinator is your permanent point of contact – from case notification to feedback.

Directly reachable – no waiting queue

Your coordinator knows your procedures, your contacts and every reported case. They personally take responsibility for three things:

Your handover

Receives the case, clarifies open questions directly with your discharge management and keeps you informed without you having to follow up.

The care

Coordinates prescription, potential assessment, aids, medication and the care team – one responsibility instead of five.

The feedback

Reports back to you in a structured way as soon as the takeover is confirmed – your proof of seamless follow-up care.

Integrity & compliance

Cooperation on a sound basis

Trust is built on clear rules. Four principles apply to every transition – without exception.

Medical responsibility remains untouched

We prepare prescriptions professionally – the medical decision and signature rest solely with the physician. No influence, no referral compensation (§§ 299a/299b StGB).

Freedom of choice for insured persons

Clients and relatives decide freely on their care provider. We introduce ourselves – we do not impose ourselves.

Transparent network structure

We disclose our cooperation partners (homecare, pharmacy, physician network). Every collaboration is aligned solely with the interests of the insured.

GDPR-compliant case handover

Client data is shared only after consent – via secure transmission channels and documented processes.
